WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?

I am a cash based service so I do not accept insurance.  I can provide you with an invoice to submit to your insurance on your own as an out of network provider if you have that benefit.  The invoice has all of the necessary information (business name and address, tax ID, national provider identification, license numbers, etc.) as well as the patient’s ICD-10 (diagnosis) and CPT (billing) codes. You may choose to submit bills following each visit, one time per month, or at any other interval, typically up to one year following your treatment visit.

Please contact your insurance company to determine your eligibility.

DO I NEED A DOCTOR’S REFERRAL?

You do not require a physician's referral to receive physical therapy services in the state of MA.  If you are choosing to submit an invoice for services to insurance for reimbursement, they may require that you have a prescription from your doctor.  Contact your insurance company for details on coverage/reimbursement.

WHAT IS CONCIERGE PHYSICAL THERAPY?

Concierge physical therapy allows you to take control of your recovery.  I can see you faster than most out-patient clinics at a time and location that works for you.  You will receive 1 on 1 services with my full attention at each session.  This means more progress with each visit resulting in fewer visits overall.  Given high insurance deductibles and co-payments, my cost will likely come close to what you would pay in a traditional clinic with desired outcomes in less time.
